Apply now by printing out The Ohio Education Trust Beneficiary form. Applications are viewed by the Board annually.
Remember the Parent/Guardian must meet federal poverty guidelines in order for their child/children to qualify as beneficiaries of the Trust. If, you are uncomfortable with putting sensitive information on the form, please wait until your follow up appointment. We are working to convert this form online, please be patient with us.
